Pregnancy & Parenting TTC can I get some help? Jan 23 2009

take 220 and subtract your age (190). Thats your maximum heart. you might want to do something like maybe 65% of that which is 124. A more rudimentary way of assessing whether your heart rate is  is adequate when exercising is the talk sing test. When our heart rate increases the demand for oxygen increases to perfuse the heart muscles so we breath faster. If you can sing while on the treadmill your heart rate isnt high enough. If you can't hold a relaxed conversation while on the treadmill your heart rate might b too high. If you are on one of those new treadmills you can input your target heart rate. They usually ask for your age and gender. While you are running you can put your hands on the bards every so often. they usually have sensors that can assess the pulses in your fingertips and tell you what your heart rate is.

Also, if you decrease your normal caloric intake by 500 cals a day, you will lose 1-2 lbs a week depending on your metabolism. The average person will lose 1 lb in a week if they decrease their caloric intake by 3500 cal in one week (500 cals a day). if you are exercising while you are dieting, this will speed up your metabolism and you will start to lose 2 lbs a week.

Health & Support I think my belly button piercing is infected, help please? Jan 23 2009

If you have any of these symptoms more than likely there is an infection:

-redness

-warmth at the site

-swelling

-pain

-scabbing or pussing

It could just be simple inflammation cause by irritation.  My advice... first, you might wanna go the the place where you got it pierced. They should know how to deal with these complications, if not... don't get another piercing there. If you think it is serious go to your primary care provider and get it assessed because inflammation that goes though the skin can get into the blood stream and inflammation in the abdomen can cause peritonitis or sepsis which isnt a good thing at all so the earlier you treat this the better. More than likely, if its just superficial the doc will prescribe you a topical or oral antibiotic.

Pregnancy & Parenting Any cures for pregnancy constipation? Jan 23 2009

I had my OB/GYN clinical rotation last semester, I'm a senior nursing student, and we would tell our mothers to make sure that they exercise along with their fluid and fiber intake. Constipation in pregnancy is most often due to the baby putting pressure in the GI tract and nerves inhibiting peristalsis. Exercising increases peristalsis. Also, laying down in bed on your left side or with a wedge under your right hip helps shift the baby off your major vessels with help perfuse the GI tract. You can also try getting on your hands and knees to reduce the pressure that on the GI tract. Have you been to any prenatal classes or birthing classes. I sat in on one last semester and they had a whole bunch of natural techniques for almost all the situations that the different mothers (and husbands) were experiencing during the perinatal period.

ALSO::: One of the most common side effects of prenatal pills is constipation do to the high iron content. Don't take laxatives unless the doctor has specifically told you too because it can cause dehydration, electrolyte and fluid imbalances...

My grandmother quit cold turkey one day after having eight consecutive heart attacks in one day in the very hospital I work in now.  She smoked a carton of cigarettes a day back then.

It only lasted for about a year, unfortunately.  She went back to them when she didn't see her health improving and figured it hasn't killed her yet, so she'll press her luck.

Wow a carton a day?!!  There are 10 packs of cigarettes in a carton and 20 cigarettes in a pack.  That's 200 cigarettes a day!  I hope that was a typo.  Surprised

I don't refer to myself as a smoker or non-smoker...I'm a habitual quitter!  Ready to quit again.  I find I last longer in the no smoke zone when I go cold turkey.  All I need is 3 days to pretend I have the flu and sleep it off.  Hard part is the pretending I have 3 days.  The patch for me is just another addiction.  I've been smoking and quitting for almost 30 years......dumb.  This time around, when I get back into the no smoke zone I'm staying there for good.  Withdrawal is not fun!

Its common for people to smoke more than 5 packs a day when they have been smoking for so long because they become so hypoxic that the smoke helps them breath... which is why you  cant give a chronic smoker 100% oxygen or they will stop breathing. It be good to figure out her pack years ( the number of packs/day times the number of years smoking). I had a COPD patient who was a 432 packyears smoker and still smokes

Health & Support Is a low-fat, high carb diet bad for you? Jan 14 2009
20:58 (UTC)

Exercise is always the best way to lose weight, when diet is used in conjunction with exercising the results are potentiated. Your diet should be governed by the type of exercises you do. For example, I am a long distance runner and I am also a male cheerleader which involves a lot of weight lifting. During running season I need the sustained source of energy to run our 13 mile races so my diet is low fat 50g of protein and I aim for 500g of carbs. During cheer season when I am weightlifting, I eat low fat 126g of protein and 300g of carbs. Carbs are good for things like cardio work outs and long extended exercises and protein is good for weight lifting and for building muscles. I try to keep my fat at 20% of my caloric intake.

Zyprexa does make you gain weight and a lot of people to to lose weight when they come off of it because of appetite suppression. I am a 22 year old college student as well and I can barely find time to eat a full breakfast lunch or dinner but what I discovered is that its good to carry around a lot of snacks with you. say fruits, veggies, protein bars, a bottle of boost plus or ensure plus. I set my alarm on my cell phone for 2 hours and I eat every 2 hours between meals. I will eat an orange, or I will eat an apple with peanut butter on it for the added fat and calories. I drink 3 bottles of boost plus or ensure a day. I find that since I am alway eating every 2 hours, my servings for lunch and dinner is smaller but I have gained more weight this way. I have gained 14 lbs in 2 weeks. I just like to add stuff t my food to increase the calories. Like yogurt with my fruits and veggies or cottage cheese with my fruits. I tend to not be hungry a lot but this averages about 3,300 cals this way and I dont feel over stuffed. Plus with this method its easy to eat anywhere you are because you can pull out a banana, apple, ensure, or protein bar in class or at the gym or in the mall. If every 2 hours is to much try 3 first. But this has definitely worked for me. Also, I increase my protein intake by making protein shakes. I actually invented my own recipe. You take a blender, add 1 bottle of ensure plus, 1 banana, a few strawberries, and a scoop of the powder protein shake mix. since i substituted the ensure for water or milk and added the banana and strawberries I increased the nutritional value. This is pretty filling and I tend to have this for breakfast and its a really fast meal. its about 550 cals depending on what protein formula you use. you have 2 of these twice a day and bam, 1/3 of your daily intake.

I just finished my Maternal-Newborn rotation last semester so I am going to give you what I tell my patients about dieting during lactation. This is a common question in women. First off, you want to make sure you are eating 300-500 cal more than you were eating when you were pregnant since you are lactating. Don't worry about adding on weight by adding these calories because most of it goes into your breast milk and provides nutrition for your infant. Carbohydrates should be 50% of your total calories (take your calories and multiply it by .5, once you get this number divide that by 4 since 1 gram of carb is 4 cals). Breast milk has carbs in it that is essential for newborn development. If you eat enough carbs, protein will be spared in metabolism which is good because it is needed for infant tissue development. Pregnancy and the postpartum period is the worst time to go on a diet if you are breast feeding because breast milk is essential for brain development. You body modifies these fats in a way that makes it essential for the brain, hence why breast fed babies have a higher IQ than formula fed infants during the first 6 months of life. Also, your breast milk is custom tailored to your infant. Your protein intake should be 1.1 times your weight in kg, thats roughly 66.22 kg so you should be getting at least 72.9 Gg of pro. A normal individuals protein intake is 0.8 x their weight in kg.

This is straight from my textbook: Protein 20% (at least 1.1 x wt in kilograms), fat limited to 30%, carbohydrates 50%. Also 8 to 10 glasses (8 oz) of fluids/day. Limit caffeine to less than 300mg/day. eat good sources of folic acid, iron, and vit C and avoid fish with high levels of mercury.

A lot of women differ in how they lose weight and their nutritional needs but this is just the standard information that we use when formulating a care plan for our labor and delivery and postpartum mothers.

The Lounge Hang-over Remedies needed...now Jan 14 2009
20:03 (UTC)

There is no remedy for hang-overs. For some people they can do certain things to mask the symptoms but that is so individual that you have to discover that on your own. Hangovers are do to a build up of acetlyaldehyde in the blood, which is the by-product of alcohol metabolism. WHen people drink so much they tend to have to urinate a lot and they might notice its clear like water. THats because it is pure water. when you break down alcohol you get acetylaldehyde and water. Our body excretes the water. Acetylaldehyde is irritating to the brain. It gives you headaches and makes you eye hurt in bright lights and irritates the vomiting center making you nauseous. FOr most people, things like pretzels and nuts help with the nausea and coca cola helps with the headaches. It doesn't make the hangover go away, it just makes it more bearable.

Depends on the patch you use. People who use patches that act on the pleasure center in the brain have more side effects, with wellbutrin. If you take the nicotine patch, you don't rally have as much side effects because you are getting the dose of nicotine you get when you smoke. what people fail to realize is that the nicotine patch wont help you quit if you keep it at the same does. If your prescriber slowly titrates the dose of the patch down, your body adjusts to it to the point that the does is so low you dont need the patch anymore... its the same principle as putting heroin addicts on methadone.

Weight Loss Does anyone else drop weight all at once? Jan 14 2009

How much do you initially weigh? A decrease in more than 10% body weight without trying isn't a good sign. Especially in such a short period of time. And it could be water weight. SOme times your retain water and sometimes you lose water. 10 glasses of water a day (depending on if you are using a standard cup which is 8 oz of a glass which can range from 10 to 16 oz) is roughly 2400 ml... that is roughly 5 lbs of water a day. If you arent losing as much water as you are drinking then it can be water weight. People with kidney problems do tend to fluctuate in weight do to water retention secondary to renal blood perfusion. If your kidneys aren't getting enough blood it thinks your blood status is inadequate and will retain water, once it gets the perfusion it needs to dumps water. Our kidneys is the main way we balance fluid and electrolyte status. I'd say subtract your initial weight from your new weight and divide that by your initial weight and multiply that by 100. that gives you your percentage gained or lost. if its 10% or more then you might want to consult your primary care provider. It could just be a dietary problem, maybe do to medications, or could be another underlying cause. Its always good to be on the safe side and double check with your MD

ALSO: You want to make sure you weigh yourself at the same time each day because different things like fluid status, what we ate, out level of activity prior to weighing ourselves can effect our outcome. Morning weights are the most accurate especially when we get right out of bed after using the washroom for the first time in the morning because we have digested mostly all of the food in our body, we are close to our resting metabolic rate, and we have just dumped fluid from the body.

Fitness Exercise for one month, gain 5lb...why? Jan 08 2009
16:52 (UTC)

muscle weighs more than fat. WHen you gain muscle mass you increase the water carrying capacity. remember 1L of water weighs 2.2 lbs (1kg). Because glucose is the main source of energy, when we start exercising we don't burn fat right away. usually you gain weight because you are building muscle but you aren't burning fat right away. As muscle mass increases, energy needs increase so you will start to burn fat. Also, a rule of thumb for nutrition is to lose 1 lb you have to decrease your diet by approximately 3500 kcalories. They suggest decreasing your daily diet by 500 kcals a day and at the end of a week you should have lost 1 lb. Most of this information is things I have learned in my nutrition and nursing courses so I can validate it.

I think for the most part, your metabolism is genetic. Also, I don't think you can change your metabolism just by the amount of calories you eat. I am always told I have a fast metabolism. I am a 22 year old make, 5'10 and weigh 135-140 lbs on and off. I eat about 3,500 kcals a day on average. I did research on trying to slow my metabolism down. Basically I would fast for a while and eat a lot of calories and then fast for a while again. I would exercise a lot while I was fasting and become a couch potato while I was eating a lot. It worked for a while, 3 months to be exact. I went up to 150, but when I started back to my normal routine and eating habits after a while I went way down to 130... now I am back up to 138. I learned that you cant permanently alter your metabolism by diet and exercising. Also, being a Nursing major, I understand the intricate neuroendocrine system and the many hormones that play a role in metabolism... the major player being thyroid hormone.

Foods Anyone not into non-fat milk? Jan 03 2009
21:19 (UTC)

WE should drink milk because unlike our ancestors, who ate a lot of dark green leafy veggies that had adequate amounts of Ca++, some of us aren't consuming enough of these dark green veggies to meet our minimum Ca++ recommendations. Having milk in our diet supplements us with more Ca++.
