putting any strain on your body (including temperature changes) will make you burn more calories than normal but it isn't hugely significant and as you get used to things like cold weather they will have let of an afftect

Have you been doing the exact same routine for a long time? Often is you repeat the same excersize regularly, your body gets used to to the motions and you stop reacting in the same way. Try to switch it up, use a different type of cardio machine every time or even use more than one per workout, also try to find some variations on the strength training you do like sitting on the bosu for some of it. 

Also, how is your diet? Working out will do nothing for weight loss if you're eating back all the calories. Try and mix something up there a little too.

a bike should be pretty easy on your feet. even the elliptical would be better than the treadmill because you don't have the impact of slamming your heel down

probably not.

If all measurements were exactly accurate then yes, you would loose weight but at a very slow pace (one pound every 2-3 weeks) However, when measuring your daily caloric burn and intake there is a pretty large margin of error, such that 200 calories would hardly make a difference

take them as usual, the absolute worst case is that you feel a tad queezy but skipping it or waiting can actually make you feel sicker and it messes with your brain's chemical balance which will seriously affect your mood. Just take it with a glass of water and something good to eat
