What is a serving size for pasta? Uncooked? Cooked?

According to the sides of most boxes, 2 ounces dry. How that translates into cooked depends on the pasta, as different types and shapes absorb different quantities of water.
The most efficient thing to do (at home anyway), is to weigh it on a scale dry before cooking.
As for eating out and estimating, that comes from experience by cooking it at home. I've learned that 1 ounce of penne pasta is about 1 cup cooked, and 2 ounces of cooked angel hair is about the size of my hands when folded (had to have some point of reference).
