Have you read about the minnesota starvation study? The results showed that if you take a perfectly healthy person and starve them then they will display the same behaviours as someone with an ED. A starved brain cannot think rationally, that is a fact. So many of the struggles people with AN have are purely the result of malnutrition. Its hard and scary YES, but your thinking really does change once you gain weight.

yes, in my teens i got stuck at the same height for years. Once i gained weight and started eating what i really should have been, and not just a 'minimum' to get by on, i grew 3 inches.

I think it depends on the person ...and actually, i do believe that putting on weight is probably the most important part. You do need support of some sort, even just from family. But you cant get better unless you want to. Until the decision comes from within nothing will change long term, no matter how much therapy you have.

I didnt make any progress until i cut off all my treatment team (not advising this) and the reason i made progress after was because i decided that i wanted to get better for me. I knew all the research and dietetic stuff, so once i decided that thats what i wanted and needed to do, i really didnt need anybody else. Family support is always nice though

Obviously some people really need and benefit from therapy, im just saying what worked for me.
