I've heard fetuccine alfredo referred to as heart-attack-on-a-plate.
A friend of mine from Texas told me about a restaurant there where the specialty is a chicken-fried bacon sandwich, served with (more) bacon, cheese and mayo.  That sounded about as bad as it gets ... at least until they figure out a way to deep fry the whole sandwich.
Well, I'll have to disagree with you on the chocolate and coffee, to a degree. Excessive is bad, yes, but it has been noted by a few articles and some threads here that a small amount of dark chocolate and coffee (sans sugar and cream) is good for you.  Something to do with antioxidants and other effects on cholesterol.

Anyhoo, I'm going to have to go with generic fried foods as one of the biggies - especially ones that are batter dipped.  Doughnuts are in that category as well, but I'm including french fries, fried chicken, fried cheese, fried fish...you get the picture. 

TGI Friday's now even has an appetizer that is fried macaroni and cheese - breaded of course.  However, the truly bizaare thing off their menu are the fried green beans.  Yeah - they battered green beans, and deep fried them.  [sighs, shakes head]
Have you ever watched the movie Spanglish...if so...do you remember the sandwich Adam Sandler makes himself (it's during the first half of the movie). Well that sandwich has inspired myself and a friend to make our own rendition of it. We have only made it about 3 times but we also refer to it as heart attack on a plate. We make it and then watch the movie. Sandwich = 3 pieces of bacon, patty of sausage, 2 eggs sunny side up all on crusty white bread....ummm is it yummy!
I think deep fried ice cream and candy bars tops the list for me. I've never tried it because it seemed to odd and grosse. I think cheeze whiz is nasty it may as well be plastic.
Not in order of importance.  They should all be avoided.

1.  Cured, salted meats that contain nitrates or nitrites (cancer causing) including hotdogs, bacon, ham, sausage, etc.

2.  Partially hydrogenated fats

3.  Deep fried foods

4.  corn syrup

5.  Lard and fatty meats

Some of what I've listed are food ingredients.  Read those labels carefully!
Cheesecake! *shudders*

Triple bacon cheeseburgers. Or any cheeseburger with multiple patties. Or pretty much all fast food in general.

Fettucino alfredo or things with really creamy sauces

Onion rings.
You can deep fry ice cream?! How on earth does that work?! Surely it melts... Or was that a joke?!
Deepfried twinkies look DISGUSTING! That would definetly be on the top of the list! One time my friend (who is a twig) bought one at the boardwalk, and I couldn't watch her eat it! Not only was it the size of her head but it was coated with butter, chocolate, sugar, and lard! It was sooooo gross!

Cinnabon cinnamon rolls are supposedly horrible for you.... but so good!......... and deep fried candy bars are scandalous looking too!

I heard that there is an amusement park somewhere that sells bacon cheeseburgers but put them between two frosted donuts instead of a regular bread bun!  Thats gotta be up there too!  Ewww!
just about anything you can get at a carnival. they will fry ANYTHING, no joke.

fettucini alfredo, fried twinkies, oreos, etc. cheesecake, cold stone's (even though i love it. i'm saving it as a treat for my birthday), soda, pastries, hotdogs, many "specialty" hamburgers, panera sandwiches, deep fried anything, bacon, mayo in general, doughnuts. God, there's a lot.
Any trans fats, "Fried Coke" [look it up on Wikipedia, I almost barfed] and butter.

Fried coke?  Wow.  I 've heard of fried snickers bars, haha, and heard they're tasty, but never really wanted to try one.  I'm not so sure why you list chocolate and coffee on your list, but I guess that's why it's your list.

I think that the worse things are those that I love, love, love, crave and miss but know that they are bad for me: pizza, mac and cheese, chinese food (sodium!), any pastry under the sun, bagels with lox and cream cheese (I'm starting to wimper), Eastern Carolina - style barbeque with hushpuppies, pasta with gorgonzola cream sauce (wimpering is getting louder, close to sobbing!), steak fries with lots of ketchup from my favorite neighborhood restaurant/bar just down the street, chicken and cheese enchiladas with mole (not the rodent but the chili/nut/chocolate mexican sauce - see what not having an accent mark for punctuation on the e in mole can do!). Ok, I think I need to go for a walk to rid my mind of temptation!
Not a Joke, though it should be. They have it at the exhibition here (fair or carnavle), actually they might have it at the mall too maybe....I cant remember.
my # 1: Caffeine drinks, like Red Bull, Ballz, RockStar, etc... I just dont think that that much caffeine is good for anyone.
Deep-Fried Oreos...but MAN are they good. 

Basically alot of the food they serve at Musikfest..Funnelcake can't be too good for ya either..all the fatty German foods too.

Doughnuts I agree on..very bad.

Tiramisu...after realizing it has like 1000+ cals..I try to stay away..but it's my favorit dessert! :(

Right up there with tiramisu is cannoli - there's this great Italian deli near me that makes them fresh - I think it's like 3/4 of a day of calories for me....

i dont think any food is bad, moderation is key. People ate butter, lard and fats as a survival technique, and they didnt die from obestiy and heart attacks, Its only the gluttony and excess of our culture, thats all to blame.

i agree that trans fat and all super refined sugars are bad, but agian, in modetaion is key.......
